The DEBORAH Way of Life plaques consist of signatures of individuals who contribute $100.00. Their name will be prominently and permanently displayed throughout the DEBORAH Heart and Lung Center.

Option 1:

If you are ordering a plaque for yourself or a family member and wish to use an original signature please press the printable form button. This will give you a two-page form to print off and mail with your payment to your local regional office.

Option 2:

If you are ordering a plaque for someone who has passed away or you are unable to get an original signature, please press continue. The name will automatically be printed in a block format. Sample – Edith Smith.
