Directive on Chapter Membership

A major function of the Membership Chairperson is to stimulate the membership at large to enroll new members. Membership is open to anyone without regard to race, color, national origin, religion, age or handicap.

A member in good standing is defined as a person whose dues are paid in full for the current year (Jan 1 – Dec 31).

Members must pledge to devote themselves to promoting awareness and financial support to Deborah Hospital Foundation in support of Deborah Heart and Lung Center. An individual may choose to become a member of more than one Chapter upon payment of dues in said Chapters. Anyone can become a member of your Chapter regardless of where they live, unless your Chapter is within a “closed community.”

  1. Dues – The minimum annual dues, in accordance with our National Bylaws, is $15.00 PER MEMBER, PER YEAR. Dues for members under 18 years old is no less than $10.00 per year.

Any member who has not paid dues for a period of two (2) years must be removed as a member and a list of such lapsed members is to be sent to the Regional Office.

  2. Upon payment of dues the new member will receive a pin and an annual membership card, to be presented at a future Chapter meeting.

  3. Paid-up Membership Function: No more than 25% of members’ dues may be expended for the cost of a membership function. The individual members must subsidize any cost over the 25%.

  4. Membership Chairperson responsibilities:
    • Complete appropriate membership cards and maintain current records of all members. A list of all current members must be made available to all Chapter Board members. Deborah Chapter membership lists are the exclusive property of Deborah Hospital Foundation. They must not be shared with any other individuals or organizations. Quantities of up to five (5) lists can be requested through the Business Office. For requests of more than five (5) lists, a Print Shop Supply Order Form must be completed.
    • Recruit and retain new members. Send Welcome to Deborah Brochure and/or Letter, and Membership Information to new members. (Include your name and return address on the form.)
    • Keep in touch with the membership at large.
    • Take attendance at all meetings.
  5. Life Membership:
A member may opt to become a Life Member for a one-time fee of $300 payable within a year of application. Each Life Membership pertains to one Chapter only, however, it may be transferred to another Chapter at the member’s discretion at any time. Life Members who want to join another Chapter as a general member must pay the regular annual dues to the second Chapter. Life Members cannot be deleted from the Chapter Membership List unless specifically requested by the Life Member.

Life Membership entitles the member to a Life Membership Card, Life Membership Certificate (Addendum D) and a Life Membership Tac or Pendant. These should be presented in a special ceremony at a future Chapter meeting.

  6. HeartBeat of Deborah Program (previously Regional Membership Program) The HeartBeat of Deborah Program is suggested for those individuals who want to support Deborah’s cause but cannot commit the time to Chapter involvement. Help make it possible for everyone to support Deborah. Inform anyone that you meet who cannot join a Chapter about the HeartBeat Program. Annual contributions are $25.00. Participants receive current information from Deborah, including the Deborah News, a free subscription to Pulse!, Clinical Update, an Annual Report and a participant card. They can become involved with any Regional activities. They can also develop a new project in consultation with the Director of the Region. Applications are available through the Regional Office.

NO DONOR CREDIT will be awarded to current members for payment of dues (i.e. general and/or HeartBeat).

PLEASE NOTE: Membership certificates to honor years of dedication are available through your Regional Office in the following categories: 5 years, 10 years, 15 year, 20 years, 25 years, 30 years, 35 years, 40 years, 45 years, 50 years.
